Reporting of Acute Hepatitis A, B and C

A recent working party representing BASHH and the HPA met to try and improve reporting of acute hepatitis A, B and C by GUM clinics and overcome the problem of patients who do not want their personal details shared.

The forms accessed by clicking here are for reporting cases of acute hepatitis A, acute hepatitis B and acute hepatitis C and are intended to be used only by GUM clinics. They can be printed off and completed by hand, or the electronic form can be completed on the computer and then printed off. They then need to be faxed securely or posted to your local Health Protection Unit. Please include as much laboratory information as possible, but leave blank those sections where information is not available. Where a patient is not willing for their name and address to be shared, please use their soundex code and first part of their postcode as well as dob if possible.

We want to make acute hepatitis A, B and C a priority and try to ensure all patients are reported.

Chronic hepatitis B and C may still be reported using existing forms according to local guidelines if appropriate.
